McCarthy has been known internationally since the 1960s for his provocative installations and performances that are often critical of society. His extensive oeuvre, which also includes drawings, sculptures, and films, is mostly inspired by myths and age-old themes in American society such as sex, power, and money. The majority of the works deal with exposing of the sublimated violence that simmers under the smooth surfaces of mass society.
